Substantive Change Forms
Addition of New Track or Degree
- The addition of a modality to a currently accredited program
- The addition of a multi-campus program
- The addition of a track targeted to differentiated career experience individuals
- The incorporation of a multi-degree option that accommodates students who are pursuing a primary degree in a different albeit related curriculum
- The addition of an offering within a related degree
Complete the New Track Substantive Change Form.
Change to Existing Track or Degree
For major changes to current track or degree, including but not limited to:
- curriculum
- degree
- competency model
- primary method of delivery (in-person, online, etc)
- decrease in required number of credits
Complete the Existing Track Substantive Change Form
Consult the CAHME Fee Schedule for Substantive Change fees.

Substantive Change Form
Complete the appropriate Substantive Change Application Form. Ensure that all sections are competed accurately and throughly. Incomplete applications may result in delays.
Submit Application
Submit your completed application form and all supporting documents to the VP of Accreditation, Eric Brichto, at ebrichto@cahme.org.
Review Process
Our review process includes:
- Preliminary review for completeness
- Detailed evaluation by the substantive change committee
- Possible request for additional information or clarification
- Detailed desk review (if necessary)
- Site visit (if necessary)
Notification
Once the review process is complete, you will receive an official notification of the decision. If approved, you will receive further instructions on implementing the change. If not approved, feedback, will be provided, and you may resubmit your application after addressing the concerns raised.
